MediaWiki should support clean URLs

Description
MediaWiki? URLs should default to something like
http://geofft.scripts.mit.edu/demo-mediawiki/wiki/Main_Page
instead of
http://geofft.scripts.mit.edu/demo-mediawiki/index.php?title=Main_Page
In order to accomplish this, we probably need to install into $addrend/w instead of $addrend. After implementing this, we also need to consider whether/how to upgrade old wikis.
